Alarm Permit Application

Submit this form when you install a monitored alarm system in your home.

If you live in the Artesia Subdivision, you are not in the jurisdiction of the Prosper Police Department. Instead, you will need to contact the Denton County Sheriff’s Office for their alarm permit application process. Additionally, if you live north of Frontier Parkway you will need to contact the Celina Police Department.

Driver's Crash Reports (CR-2 Form Also Called a "Blue Form")

The Drivers Crash Reports can be completed by a driver of a vehicle involved in a crash that is not investigated by Prosper PD. This form is also available in our lobby.

The driver of a motor vehicle involved in a crash not investigated by a law enforcement officer and resulting in injury to or death of any person, or damage to the property of any one person, including himself, to any apparent extent of at least one thousand dollars ($1,000), must within 10 days after such crash complete and forward CR-2 report in accordance with the instructions on the CR-2.

Many times our officers will be called to the scene of the accident but allow drivers to exchange information and advise them to complete this form.  More information on the blue form’s use is located on the first page of the form.

Identity Theft

Prosper Police Department provides an Identity Theft Packet that includes educational information on steps to take if you are a victim of identity theft and an Identity Theft Victim’s Complaint and Affidavit. This form is to be used for New Account or Collection Account Fraud (not check collections), Employment Fraud, and Criminal Identity Theft.

Take Me Home

The Prosper Police Department has continued to grow not only in personnel, but also in technology, training, and outreach efforts.  We are excited to offer this program to the citizens of Prosper in an effort to provide enhanced service and protection.  Take Me Home is a program that was developed by the Pensacola Florida Police Department.  It is designed to assist individuals with disabilities, special needs, or anyone who may need special assistance in an emergency situation.  Take Me Home is a free service available to any resident of Prosper.  It is aimed at individuals with Autism, Down syndrome, developmental or cognitive disabilities, as well as senior citizens whom suffer from dementia or Alzheimer’s.  All information is entered into a database and is kept confidential. The database is only accessible to public safety personnel.

Once the registration form is complete with a current description, emergency contacts, and identifying information, bring it to the Police Department along with a current photograph. We recommend taking a profile picture as well.
