How do I obtain an EMS incident report?

You may obtain a copy of an EMS incident in which you were involved by emailing Chief Bill Bonny or calling 972-347-2424 or in writing to:

Prosper, Texas 75078

Please include your name, address, telephone number, date and time of the incident, nature of the incident, and incident location in your request. You must pick up the EMS report in person and provide valid photo identification in order for the report to be released. For patients under the age of 18, only legal guardians with valid photo identification will be allowed to pick up the EMS report. Prosper Fire Rescue will comply with all court-ordered EMS document releases.
